1. Overview of Home Health
This chapter provides a general understanding of home health, from the home health clientele to the services that home health agencies provide. Also covered is the role of each stakeholder in home health: client/caregiver, agency, the public and regulators, and payors.
2. Evaluation in the Home
This chapter provides an understanding to how PTs or RNs open a case and how that affects an OT's ability to start a case. Therapists will learn how to perform a home safety assessment and how to use assessments in the home. Also included is a demonstration inside the home of a patient.
3. Documentation of Evaluation Results
This chapter covers the importance of timely documentation and provides a brief review of 30 day assessments, re-assessments, and daily notes. Therapists will understand the minimum requirements for documentation in home health and learn to appreciate the benefit of surpassing minimum requirements. The importance of communicating with the team, caregivers, and physicians is also covered.
