The first Annual Performance Reports (APR) provided home health agencies the first look into their Home Health Value-Based Purchasing (HHVBP) payment impact for 2025, and now is the perfect time to look back at the lessons learned over the past year while preparing to maximize clinical and financial success in the new year.

On December 11th, 2024 at 12PM EST, experts Mike Brents and John Rabbia will provide home health leaders with a better understanding of the Home Health Value-Based Purchasing model in both its current and future configuration. They will discuss the implications of the release of the first Annual Performance Reports, and what that means for future revenue impact. There will also be a focus on the new scoring for Discharge to Community, replacement of Total Normalized Composite (TNC) measures with Discharge Function Score, now focusing on Potentially Preventable Hospitalizations.
