Digital dislocations are common hand injuries that can have significant functional consequences if not properly addressed and rehabilitated. In this course, the anatomical structures involved in various types of digital dislocations are examined. Potential conservative and operative management indicators for digital dislocation are also discussed. Through this course, the therapist will learn to integrate safe motion parameters for conservative and postoperative management of digital dislocations as well as formulate strategies to advance treatment based on patient precautions, tolerance, and performance. Appropriate orthoses to manage various types of digital dislocations are discussed and demonstrated.

This course is recommended for occupational and physical therapists, in an outpatient or inpatient setting, looking to improve their ability to maximize patient outcomes from acute interventions to return to work and sports activities.

1. Acute Digital Dislocation Pathology, Healing, and Management

Proper acute management and protection is critical to protecting the reduction of a dislocated joint. This chapter illustrates of the stabilizers of the MP, PIP, and DIP joints as well as the effects of disruption of each of the digital joint stabilizers. Acute management strategies, including orthosis protection and surgical management of joint dislocations involving the digits, are provided.

2. Therapy Management of Digital Dislocations

This chapter describes progression of therapy for digital dislocations from the acute phase to facilitation of motion and unrestricted function. Important rehabilitation precautions are emphasized, and activities and orthoses to facilitate return to unrestricted hand use are illustrated.

3. Orthosis Intervention and Protection for Digital Dislocation

The use of orthoses to protect and facilitate safe motion and return to function are key to successful rehabilitation of digital dislocations. This chapter demonstrates key fabrication components of orthoses used to protect injured structures as well as modifications of orthoses to protect patients during return to sports or other activities after injury.
