Patient safety has taken on increasing importance in the U.S. healthcare system. Healthcare in the home poses unique challenges to assessing and preventing issues of patient safety.

Throughout this course, learners will explore the unique challenges to ensuring consistent quality of care and patient safety in home health and hospice, organizational responsibilities to prevent, track and report incidents and adverse patient events, common types of incidents and adverse events in home health and hospice, and strategies for preventing them from occurring.

3) Define and describe types of incidents and adverse events
4) Discuss strategies for preventing incidents and adverse events
5) Promote a culture of safety in home health and hospice

This course has been certified by ACHC, and verified by Community Health Accreditation Partner (CHAP), and it was designed in alignment with the CMS conditions of participation for home health organizations. It provides training on the identification, reporting, and prevention of adverse events, which home health and hospice organizations must measure, analyze, and track as part of their QAPI program as required by the following federal regulations related to home health and hospice services:
