Populations needing physical assistance after injury or insult require attention to detail to ensure successful transitions from acute care through rehabilitation and into the community. This course will focus on the needs of patients with spinal cord injury, brain injury and stroke. Education for caregivers, equipment, community re-entry and other special considerations will be presented. This course is intended to address the discharge needs of patients which should be attended to by the case manager and interdisciplinary team.

1. Coordinating for Patients with Spinal Cord Injury

This chapter will review special considerations for care transitions from acute settings of patients with spinal cord injury. Caregivers, equipment, medical/physical/psychosocial considerations and community re-entry will be discussed.

2. Considerations for Patients with Acquired Brain Injury

This chapter will present goals for successful transition of patients with brain injury through the continuum of care. Options for community settings will be discussed. The importance of and implications for caregivers will be discussed, as well as patients’ re-entry into the community.

3. Transitions for Patients with Stroke

This chapter will explain preparation of caregivers of stroke patients for successful transition to home. Education regarding safety, cognitive concerns and comorbidities will be discussed.

4. Discussion with a Caregiver

This discussion will focus on caregiver preparation. Transition to home and changes in expectations will be explored.
