The evaluation of chemotherapy-induced peripheral neuropathy (CIPN) is a topic that has not been specifically addressed in rehabilitation literature. Rehabilitation for CIPN usually relies on evaluation strategies that are traditional in our field but are not necessarily suited to the unique clinical characteristics of CIPN. For example, a patient with a peripheral nerve injury does not have the same presentation as a patient with CIPN. This course provides evaluation recommendations and videotaped demonstrations to enable practitioners to select meaningful tests and interpret the findings using knowledge specific to the diagnosis of CIPN. Evaluation choices will be explained and demonstrated to maximize rehabilitation success. This course is informational to nurses and applicable to occupational therapists and physical therapists in a variety of settings, including inpatient and outpatient hospitals, outpatient private practices, skilled nursing facilities, long-term care, home health, and hospice environments.
Learning Objectives
- Select appropriate clinical tests to evaluate an adult patient with CIPN
- Discriminate rationales for the use of the Ten Test versus the two-point discrimination test and/or the monofilament test in adult patients with CIPN
- Distinguish between sensory pain and sensory impairment
- Administer the use of the Ten Test for sensation in the hand
- Execute three gentle, provocative maneuvers of the upper extremity
- Correlate findings of provocative upper extremity tests with clinical observations and sensory test results to determine the impact on ADL and IADL performance

1. Overview of CIPN Clinical Evaluation
This chapter offers helpful CIPN evaluation screening questions, presents popular scales of CIPN symptom severity, and identifies strengths and weaknesses of these tools. This is important because many oncology teams use these scales in their research and treatment.
2. Sensory Evaluation
This chapter explains the difference between sensory qualities of pain versus impairment, presents sensory testing procedures and norms, and demonstrates the Ten Test. This material provides the practitioner with choices for establishing the sensory baseline and sensory changes with treatment.
3. Upper Extremity Provocative Tests in Patients With CIPN
This chapter demonstrates upper extremity provocative tests appropriate for CIPN patients. Emphasis is placed on being gentle and the importance of not eliciting pain. Videotaped demonstrations are included. This is important because it allows practitioners who are not familiar with CIPN evaluation an opportunity to see what the hands-on testing looks like.
