This short introductory course explains what Medbridge Pathways is and how it supports team-based hybrid care. Designed for clinicians and care coordinators, the course outlines Pathways’ key features, common workflows, and the patient experience. Learners will explore how Pathways helps care teams assign structured digital care plans, monitor progress, and intervene when needed. The module also highlights how Pathways supports patient engagement, reinforces care plans between visits, and helps identify risks earlier through tracked interactions and symptom data.
Course Objectives
1. Describe how Pathways supports hybrid care through structured, phase-based digital programs
2. Identify the roles and responsibilities in usual Pathways care teams
3. Describe the patient experience with Pathways, including how they receive care plan tasks and how their progress is shared with the care team
4. Recognize how Pathways supports patient engagement, risk identification, and care plan reinforcement
This course provides a foundational overview of how Pathways supports hybrid care delivery. Additional training usually follows this course to cover the steps for assigning, managing, and navigating the Pathways tool.
