Description

Helping patients make meaningful behavior changes is central to clinical care, yet many clinicians struggle to fit effective behavior change conversations into time-limited visits. Traditional approaches often rely on education, advice-giving, or prescriptive goal setting, which may not sufficiently support patient autonomy, confidence, or follow-through. This webinar introduces Brief Action Planning (BAP), a feasible, structured, and patient-centered approach for helping patients create realistic action plans for behaviors such as home exercise, physical activity, nutrition, sleep hygiene, and other health goals. Participants will learn the spirit and core skills of BAP, including offering behavioral choices, eliciting SMART action plans, assessing confidence, problem-solving barriers, and supporting follow-up. Through practical examples and common clinical scenarios, learners will gain language and strategies they can immediately apply in routine care. This webinar is designed for rehabilitation professionals, primary care clinicians, lifestyle medicine providers, health coaches, and other clinicians working in outpatient, community-based, preventive care, chronic disease management, or rehabilitation settings.
