Even when clinicians recognize the importance of behavior change, many patient goals remain too broad, vague, or clinician-directed to support meaningful follow-through. Practitioners may advise patients to exercise more, improve nutrition, or perform home exercises, but lack a reliable structure to help patients convert those recommendations into small, specific, realistic action plans. This 90-minute webinar teaches the core skills of Brief Action Planning (BAP), including offering a behavioral menu, eliciting SMART action plans, inviting commitment statements, and using confidence scaling to strengthen self-efficacy. Through practical examples and patient-centered language, participants will learn how to help patients choose actions that are feasible, meaningful, and aligned with their daily lives. This course is intended for rehabilitation providers, physical therapists, occupational therapists, primary care clinicians, nurses, health coaches, wellness professionals, and others working in outpatient, community, preventive, chronic disease, and lifestyle medicine settings. Learners will gain repeatable tools they can immediately use to make behavior change conversations more structured, efficient, and effective.
