Patients often leave clinical visits with well-intentioned action plans but encounter predictable barriers such as low confidence, pain, fatigue, time constraints, caregiving responsibilities, cost, access, or disrupted routines. Practitioners may struggle to respond when plans are only partially completed or not completed, and follow-up conversations can unintentionally feel like accountability checks rather than opportunities for collaborative problem-solving. This 90-minute webinar focuses on how to use Brief Action Planning (BAP) to troubleshoot barriers, adjust plans, normalize setbacks, and structure follow-up in ways that preserve patient autonomy and build confidence. Participants will practice applying the full BAP sequence to realistic clinical scenarios and learn practical implementation strategies for busy care settings. This course is designed for rehabilitation professionals, physical therapists, primary care clinicians, nurses, health coaches, community health workers, and wellness professionals working in outpatient, community-based, preventive, chronic disease management, rehabilitation, and lifestyle medicine settings. Learners will leave prepared to help patients recover from barriers, refine plans, and sustain progress over time.
