Cindy Krafft, PT, MS, HCS-O

Cindy Krafft brings more than 20 years of home health expertise that ranges from direct patient care to operational and management issues. Years spent in the homes of patients confirmed that she was in the best setting to focus on functionality and the specific challenges faced … by each patient. Cindy recognizes that providing care in the home environment is different from providing care in any other setting, which is evident in both her training and consultation activities.

For the past 10 years, Cindy has been a nationally recognized educator in the areas of documentation, regulation, therapy utilization, and OASIS. She has served in several national projects and been an expert resource for OASIS updates. Her focus is on providing the knowledge and tools to operationalize external requirements while keeping the driver of care where it needs to be--the needs of each patient being cared for in the home setting.

Cindy has been involved at the senior leadership level of the Home Health Section of the American Physical Therapy Association for more than 10 years and is the immediate past president of that organization. She has been working with APTA and CMS to clarify regulatory expectations and address proposed payment methodologies to ensure the long-term participation of therapy services in home health. She has written two books--The How-to Guide to Home Health Therapy Documentation and An Interdisciplinary Approach to Home Care--and coauthored a third, The Post-Acute Care Guide to Maintenance Therapy.

Joseph Brence, PT, DPT, MBA, FAAOMPT

Head of Clinical Strategy at MedBridge

Joseph is an energetic healthcare innovator and leader with a demonstrated track record of clinical excellence, specifically focusing on strategies to transform the delivery of post-acute care. Joseph is currently employed by MedBridge as the Head of Home and Community Care Strategy … and Solutions. Prior to this role, he focused on innovation, strategy and education within the post-acute division of a large IDFS. Joseph is an equity partner at the Nxt Gen Institute of Physical Therapy and sits on the advisory board for the Rangos School of Health Sciences at Duquesne University. He has assisted with national healthcare strategy efforts at the American Physical Therapy Association, as well as the Department of the VA. Joseph is also a faculty member at NYU as a course lead for entrepreneurship and strategic management.

Sherry Teague, MESS, ATC, PTA, HCS-D, HCS-O

Sherry, a native Floridian, is the CFO and co-owner of K& K Health Care Solutions. Sherry has been the driving factor in both her and Dee's movement from employment to ownership - first with a contract rehabilitation company (Integrity Rehab, Inc.) that provided physical … therapy, occupational therapy and speech-language pathology services to local home health agencies in Marion and Citrus counties, and then into their own home health agency, founded in 2003. In 2009, Sherry was honored with the Marion County Chamber of Commerce "Small Business Person of the Year" Award. Sherry is dedicated to taking the pain out of continuing education for health care clinicians and administration by leveraging technology and meeting customer needs. Sherry has a strong sense of community service, as evidenced by her volunteer status as an athletic trainer at Citrus High School for over 13 years, as well as her membership with Rotary International that exemplifies service above self. Sherry received her athletic training certification from Valdosta State University (GA) in 1989, and her Masters in Exercise and Sport Science degree from the University of Florida in 1991. She completed her A.S. degree at St. Pete Jr. College in St. Petersburg, FL, to become a physical therapist assistant in 1998.
