Charles M. Breznicky Jr.

Charles M. Breznicky Jr. has been part of SimiTree Healthcare Consulting since 2015 and has been a registered nurse since 2004. He obtained dual master's degrees in nursing administration and in business administration from LaSalle University in 2014 and obtained his HCS-D certification in 2021. Prior to joining SimiTree, he provided direct patient care in the acute care setting and worked as a home health field nurse.
Charlie has served in various roles in the home health industry, including performance improvement and compliance specialist, clinical manager, and health information systems manager. He is currently the clinical director of SimiTree and has assisted agencies in developing QAPI programs, preparing for surveys, and adapting to the recent changes to the Home Health Conditions of Participation and the Patient-Driven Groupings Model.
