Hospice care can be provided in an assisted living facility (ALF). Providing hospice care to patients in an ALF is similar to providing hospice care to those patients if they lived in a private residence. However, there is a need for coordination between the hospice care team, patients, families, and ALF staff to ensure that patients receive the care they need. During this course, we will discuss the provision of hospice care in an ALF and the coordination that must occur between the hospice team and the ALF staff to ensure optimal care and patient safety.

Chapters & learning objectives
1. The Hospice and Assisted Living Facility Partnership
Hospice care can be provided in an assisted living facility (ALF) per state-specific ALF regulatory allowance. This chapter discusses hospice care provision and the partnership between the hospice team and the ALF staff per state regulatory leeway.
2. Requirements for Providing Hospice in an ALF
There are no federal regulations for ALF operation, but there are state licensing requirements. This chapter explains how a hospice and ALF provider work together for the provision of hospice care per an individualized plan of care when a patient resides in an ALF.
3. Coordination of Care for the Patient/Family
This chapter discusses the necessary coordination between the hospice team and the ALF staff to implement the patient’s plan of care. This coordination of care is demonstrated with a case study involving a patient who opts to have hospice care while living in an ALF.
