This chapter will discuss the risk factors of developing diabetes and the diagnostic tests used to assess and manage risk factors and diabetes.
2. Geriatrics 5Ms and Diabetes Management
This chapter will provide the learner screening tools to address mentation, medication, mobility, matters most, and multicomplexity for the patient. Additionally, the learner will integrate risk factor management into care planning.
3. Integrating Chronic Care Model Principles Into Care Planning
The participant will integrate patient-centered care into exercise prescription and patient education to facilitate behavior change and sustainability of change.
