Occupational Therapy and Cognitive Behavioral Approaches

Presented by Stephen Nawotniak

Video Runtime: 44 Minutes; Learning Assessment Time: 31 Minutes

This introductory course explores the application of evidence-based cognitive and behavioral approaches in occupational therapy practice. While cognitive behavioral therapy (CBT), dialectical behavior therapy (DBT), and acceptance and commitment therapy (ACT) are well-established interventions in mental health, this course bridges the gap between theory and OT-specific practice. Participants will examine the foundational concepts of each approach and learn practical strategies for integrating these cognitive-behavioral principles into occupational therapy treatment planning and delivery.

Learning Outcomes
  • Differentiate how thoughts, feelings, and actions interact within the cognitive triad to guide occupational therapy intervention planning
  • Select integrated CBT-informed occupational therapy strategies that utilize valued activities to address unhelpful thought patterns and mood
  • Classify DBT-informed occupational therapy interventions that integrate mindfulness, distress tolerance, and interpersonal skills into daily client activities
  • Clarify the core principles of Acceptance and Commitment Therapy (ACT), including psychological flexibility and the clinical application of the ACT Matrix
  • Predict how ACT-informed occupational therapy interventions support valued living by engaging clients in meaningful activities despite internal discomfort or perfectionistic thoughts

Chapters & learning objectives

Intro and Cognitive Behavioral Therapy (CBT)

1. Intro and Cognitive Behavioral Therapy (CBT)

This foundational chapter introduces cognitive-behavioral approaches as evidence-based interventions, positioning them as mental tools for addressing challenges in treatment and recovery. Learners will examine the core principles of CBT, including the cognitive triad—the relationship between thoughts, feelings, and actions—and explore how this framework has developed over time. The chapter establishes the theoretical foundation that supports the practical OT applications presented in subsequent chapters.

Dialectical Behavior Therapy (DBT)

2. Dialectical Behavior Therapy (DBT)

This chapter focuses on dialectical behavior therapy, a structured, evidence-based approach originally developed for individuals with complex mental health needs. Learners will identify the core components of DBT and explore how occupational therapy practitioners can integrate DBT principles into treatment activities and interventions. The chapter bridges DBT theory with practical OT implementation, demonstrating how OT practitioners can support clients in developing dialectical thinking and behavioral skills within their occupational contexts.

Acceptance and Commitment Therapy (ACT)

3. Acceptance and Commitment Therapy (ACT)

This chapter introduces acceptance and commitment therapy, a third-wave cognitive-behavioral approach emphasizing valued living and psychological flexibility. Learners will examine the key components of ACT and become familiar with the ACT matrix as a clinical tool. The chapter demonstrates how occupational therapy practitioners can apply ACT principles to help clients clarify values and align occupational engagement with meaningful life directions, offering practical strategies for incorporating ACT-informed interventions into OT practice.
