Medication-related problems are common among patients, especially those who have multiple disease states and complex medication histories. The management of patient medications can be a challenge for all health care practitioners in both the hospital and home care setting. This course will review the steps of the medication reconciliation as discussed in Part One of the series, titled Home Care Medication Management: Performing a Medication Reconciliation. It will include a detailed discussion of the barriers to an effective medication reconciliation and other areas in which the process may fail. This is followed up by how to overcome these hurdles and strategies to improve the process. A relatable case review is provided. This course is applicable to nurses, therapists, social workers, and any other health care professional interacting with patients in home health and/or hospice settings.

1. Course Overview

This chapter discusses the motivation to take this course, followed by a course roadmap and learning objectives. There is a short review of the basic steps of a medication reconciliation.

2. Barriers to Effective Medication Reconciliation

This chapter discusses the barriers that leads to an ineffective medication reconciliation.

3. Strategies to Improve a Medication Reconciliation

This chapter strategizes and examines ways to improve the medication reconciliation process. Included is an analysis of some medication-related red flags to keep in mind and why these are important factors. There is also a brief introduction of the Beers criteria and how it can be used during a medication reconciliation.

4. Putting It All Together

This chapter analyzes a case scenario to apply what was learned in this course. Each step of the medication reconciliation process is broken down, reviewed, and applied to the particular patient being discussed. The Beers criteria is also used and applied to the case scenario. Finally, there is an overview discussion with some key takeaway points.
