In early 2021, CMS announced its expansion of the Home Health Value-Based Purchasing model (HHVBP) beginning in 2022, while also increasing the maximum positive and negative payment adjustment to eight percent. Whether you’re already operating under the HHVBP model or will be adopting it for the first time following the expansion, your home health agency will need to be prepared for the myriad of changes in order to achieve and maintain quality outcomes.

Charles M. Breznicky Jr., RN, MSN, MBA, HCS-D, has been part of SimiTree Healthcare Consulting since 2015 and has been a Registered Nurse since 2004. He obtained dual Master's degrees, Nursing Administration and Business Administration, from LaSalle University in 2014 and obtained his HCS-D certification in 2021. Prior to joining BlackTree he provided direct patient care in the acute care setting as well as working as a home health field nurse. Charlie has served in various roles in the home health industry including, Performance Improvement and Compliance Specialist, Clinical Manager, and Health Information Systems Manager. He is currently the Clinical Director with SimiTree and has assisted agencies to develop QAPI Programs, prepare for surveys, and adapt to the recent changes to the Home Health Conditions of Participation and the Patient Driven Groupings Model.
