Jim Rivard

PT, MOMT, OCS, SCS, FAAOMPT

Jim Rivard received his Bachelor of Science in Physical Therapy from the University of Puget Sound in 1988, after which completing four years of postgraduate residency and fellowship training from the Ola Grimsby Institute (OGI) and a Chronic Pain Management certificate program from McGill University. Jim is the founder of MTI Physical Therapy in Seattle, teaching full-time residency and fellowship programs. Jim is also the program director for the Sports Residency program at MTIPT. As a Certified Orthopaedic and Sports Clinical Specialist, Jim continues to practice clinically, regularly working with high school and collegiate athletes, as well as professional rugby players (Seattle Seawolves!). He has served as a board member and president of the American Academy of Orthopaedic Manual Physical Therapists (AAOMPT). He has also been an instructor and mentor for the OGI for more than 25 years, teaching more than 250 courses and conference presentations throughout the United States and internationally in 11 countries. The coauthor of three textbooks on exercise rehabilitation, Jim is responsible for development of the STEP (Scientific Therapeutic Exercise Progression) curriculum and has published articles on exercise design and dosage. Jim has been recognized for his clinical skill, receiving the 2016 Clinical Excellence Award from the Washington State Physical Therapy Association, the 2019 John McMillan Mennell Service Award from AAOMPT, and the Hallvard Award from the OGI.

Eccentric training for treating tendinopathies has become more deeply embedded into clinical practice over the last several years. Despite what the evidence suggests about this particular intervention, clinical outcomes do not always follow expectations. In fact, studies on clinical application do not always properly address critical details related to patient selection, dosage considerations, the role of isometrics in particular cases, and where this training fits into a broader rehabilitation program.

This webinar will review the historical context of mechanotherapy in training collagen pathologies and provide clinical decision-making models for differential diagnosis. Exercise prescription, including concepts of exercise dosage and recovery for tissue training, will also be addressed. Finally, a framework for exercise progression will be outlined, including consideration of individual impairments and variables.
