Patients requiring surgical procedures may no longer remain in the hospital for any length of time for post-operative recovery. Many are sent home and require ongoing care. Home health nurses are positioned to care for these individuals. This course will address the most common types of post-op care encountered. This course addresses assessment strategies, as well as instruction and activities utilized to minimize complications and enhance rehabilitation. Part two of this two-part course discusses care planning and applies concepts from both parts to a home health case study.

1. Care Planning and Educating Patients to Reduce Complications and Improve Rehabilitation

As many times as patients can be assessed, without appropriate care planning, education, and action, those assessments will serve little purpose. This chapter presents care plans, educational topics and activity opportunities the nurse can utilize with patients to best ensure reduction of complications and for the enhancement of an optimal rehabilitation.

2. Putting All the Pieces Together

When receiving significant amounts of information, the best retention can be achieved when we are able to implement the teaching promptly. In this chapter, we look at a case study of a home health admission and use the learning in the previous four chapters to assimilate the information and formulate a strategic and evolving care plan.
