Case Excerpt:
John Lewis is a 29-year-old male, newly admitted to the rehabilitation unit for a T-1 complete spinal cord injury. Before his injury, he practiced as a lawyer. Injured during a skiing accident, he is now depressed and anxious. He is not married, and his parents are deceased. He has one sibling who lives in another state. Mr. Lewis complains to the nurse that he has a severe headache.Question:
Which of the following should the nurse do first in response to Mr. Lewis’s complaint of headache?
The correct answer is "Reposition Mr. Lewis into a head-up, feet-down position."
A headache in a person with a complete spinal cord injury above T-6 is suspicious for autonomic dysreflexia. A head-up, feet down position is the first intervention. Acetaminophen is not indicated for this condition, which is an emergency situation. Although he may have anxiety, it cannot be assumed that anxiety is the cause of the headache. Transferring to a head down position in bed may make the condition worse.Kristen L. Mauk, PhD, DNP, RN, CRRN, GCNS-BC, GNP-BC, FARN, FAAN
